Want to print these forms to bring to your new patient appointment?

Right click on the page that you want print, (there are ten total) then select print picture and print! Bring these forms to your first visit.  There are 4 pages to our privacy policy.  We only need the last page with your signature.  Keep the other 3 pages for your records. A copy of the Privacy Policy is available at our office upon request.
